To those of you that were able to apply to the State Board for the renewal of your PHARMACY license, your new license received in the mail should be reviewed very closely.
We are finding that expiration dates on many received are incorrect. The expiration should be August 2019.
We are also finding PIC name is also incorrect. Please review closely.
